Student Referral
The student is brought up in a grade-level team meeting.
The team brainstorms 2 - 3 interventions to be implemented in the classroom and other settings as appropriate
Teacher fills in Observation Log
Teacher contacts parents with concern and classroom strategies to be tried with the student
Teacher implements and documents classroom interventions for 2 - 3 weeks
Documentation will help to track progress and determine next steps
If the student responds well to interventions the team continues to MONITOR with check-ins from learning specialist as needed
Teacher communicates with the family the child's progress and plan moving forward
Intervention Plan
The teacher works with the learning specialist to develop additional strategic interventions based on data and progress (Response To Intervention) for approx. 2 - 3 weeks
Teachers continue to document in Observation Log
Supplemental support by a learning specialist may be added to the intervention plan
Team shares outcomes with parents/guardians (All team members should be CC'd on all communications)
The Team makes recommendations for: either continued support, monitor only status by learning specialist or next steps
Examples of support may include a referral for a tutor which will be communicated through Learning Services to parents
Learning Specialist is the contact person from this point on with parents and teachers
Next steps
The team meets to decide next steps which may include an informal assessment from the Learning Services department or a referral for an outside professional to conduct a formal assessment (see Key Definitions under Evaluations for the different ones which may be recommended by the Director of Learning Services or Division Head)
The team designs an individual support plan based on students needs
Intervention may include: 1:1 or small group pull out from learning specialist or in-class support during targeted curriculum times
There is ongoing parent and team communication regarding progress
If the student is referred for outside assessments the team will meet to go over results, recommendations and additional next steps
Division Head or Director of Learning Services makes outside referral for formal testing
